Community-Based Health Care

A stay at a rehab unit, nursing home or hospital is usually temporary and it is a good idea to start planning for discharge as soon as possible. A patient might be well enough to leave the facility, but family members and others providing support might not feel ready for this move. The transition to home can be difficult for both the patient and their family member so preparation is key. The goal of any successful discharge is to prevent readmissions.
